Job summary

CHE Behavioral Health Services seeks licensed mental health professionals to provide in-person psychotherapy and behavioral health services within Skilled Nursing Facilities and Assisted Living communities.

Choose full-time or part-time schedules with clinical autonomy, while enjoying comprehensive support from an established multidisciplinary team and seamless referral relationships that foster continuity of care for residents.

Qualifications

• Active and unrestricted Pennsylvania license as one of LPC/LCSW/LMHC/LMFT.
• Master's or doctoral degree in an applicable discipline.
• Ability to provide compassionate, patient-centered care.
• Strong communication and documentation skills.
• Experience in nursing home and long-term care settings preferred.

Responsibilities

• Provide in-person psychotherapy and behavioral health services within SNFs and AL communities.
• Conduct diagnostic assessments and develop treatment plans.
• Treat geriatric mental health concerns such as depression and anxiety.
• Maintain timely and accurate clinical documentation.
• Collaborate with interdisciplinary care teams to improve outcomes.
• Build therapeutic relationships with residents and families.
