Job summary

CenterWell in Texas is seeking a Primary Care Physician to join Conviva/CenterWell Primary Care on a full-time basis. You will provide comprehensive outpatient care for adult and geriatric patients in a collaborative, patient-centered team.

This role reports to the Associate Medical Director, offers a lower daily patient load, and includes a strong benefits package with competitive compensation; 40-hour week, onsite in Jourdanton.

Qualifications

• MD or DO from an accredited medical school.
• Active, unrestricted medical license in the state of practice.
• Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
• If selected for this role, you will be required to be screened for TB.

Responsibilities

• You will deliver comprehensive outpatient care to adult and senior patients.
• You will maintain accurate medical records and documentation.
• You will diagnose and manage moderately to complex medical conditions.
• You will coordinate referrals and collaborate with specialists.
• You will participate in clinical quality improvement projects.
• You will work with interdisciplinary teams to ensure holistic care.
• You will contribute to projects and innovations in care delivery.
