Job summary

Reliant Medical Group in Central Massachusetts (Worcester County) is seeking a full-time 1.0 FTE Physician to join our Skilled Nursing Facility team. The role blends on-site facility care with remote duties where appropriate, within Optum's Care Delivery network.

You will lead a multidisciplinary team, deliver value-based care on rehab and long-term units, and collaborate with an advanced practice clinician to align goals of care.

Qualifications

• National Board Certification in Internal Medicine or Family Practice.
• Massachusetts medical license in good standing.
• Graduate of an Accredited Medical School.

Responsibilities

• Care for older adults in a skilled nursing facility setting.
• Lead a multidisciplinary rehab team including APP, nursing, therapy and social work.
• Provide value-based care on rehab and long-term units.
• Discuss goals of care with patients and families and caregivers.
• Collaborate with APP to manage patient needs and progress.
• Monitor acute and chronic conditions common in nursing homes.
• Follow patients longitudinally per state regulations.

Education

Graduate of an Accredited Medical School

Licensed by the Commonwealth of Massachusetts board of registration in Medicine

National Board Certification in Internal Medicine or Family Practice

Maintains BLS certification, professional certification as appropriate
