Job summary

Humana’s CenterWell Primary Care is seeking a Primary Care Physician to join our Tidewater clinic in Norfolk, VA. The physician will provide comprehensive outpatient care for adult and geriatric patients in a collaborative, patient-centered setting.

You’ll report to the Associate Medical Director and work on-site at the Tidewater clinic, contributing to quality improvement and care delivery innovations with a multidisciplinary team.

Qualifications

• MD or DO from an accredited medical school.
• Active, unrestricted medical license in the state of Virginia.
• Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
• If selected for this role, you will be required to be screened for TB.
• 2 years' experience in value-based care or managing high-acuity geriatric patients.
• Bilingual English/Spanish

Responsibilities

• You will deliver comprehensive outpatient care to adult and senior patients.
• You will maintain accurate medical records and documentation.
• You will diagnose and manage moderately to complex medical conditions.
• You will coordinate referrals and collaborate with specialists.
• You will participate in clinical quality improvement projects.
• You will work with interdisciplinary teams to ensure holistic care.
• You will contribute to projects and innovations in care delivery.
