CenterWell Senior Primary Care in Chesapeake, VA is seeking a compassionate Primary Care Physician to join our team. You'll deliver comprehensive outpatient care, manage complex medical conditions, and coordinate with specialists in a collaborative environment.

Qualifications

• Active, unrestricted medical license in the state of practice.
• At least 2 years’ experience in managing high-acuity geriatric patients.
• Will be screened for TB if selected for this role.

• Deliver comprehensive outpatient care to adult and senior patients.
• Maintain accurate medical records and documentation.
• Diagnose and manage moderately to complex medical conditions.
• Coordinate referrals and collaborate with specialists.
• Participate in clinical quality improvement projects.

Skills

The Physician serves as a health-care professional and able to handle a variety of health-related problems. The Physician work assignments involve moderately complex to complex issues where the analysis of situations or data requires an in-depth evaluation of variable factors.

You will work on-site at our Chesapeake clinic. You will report to the Associate Medical Director for the Tidewater Region.

Main Responsibilities

• Deliver comprehensive outpatient care to adult and senior patients.
• Maintain accurate medical records and documentation.

Language Proficiency Testing

Any associate who speaks with a member in a language other than English must take a language proficiency assessment provided by an outside vendor, as well as the Interagency Language Rating (ILR) test as provided by the Federal Government.

Pay Range

$255,000 – $298,500 per year. This job is eligible for a bonus incentive plan based on company and/or individual performance.
