Job summary

Eventus WholeHealth is seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to provide comprehensive primary care within skilled nursing and senior living communities. You will perform routine rounds, manage chronic diseases, and coordinate care across interdisciplinary teams with facility partners.

Applicants should hold current NP/PA licensure in their state and have experience in geriatrics, family medicine, or internal medicine.

Qualifications

• Current NP or PA licensure in the state of practice.
• Previous professional patient care experience.
• Experience in geriatrics, family medicine, or internal medicine preferred.
• Exceptional patient-centered service skills, including compassionate communication, active listening, professionalism, and the ability to build trusting relationships with residents, families, and facility staff
• Strong collaboration skills with the ability to work effectively across interdisciplinary teams

Responsibilities

• Provide comprehensive primary care services within skilled nursing and senior living communities.
