You will perform routine rounds, manage chronic conditions, and coordinate care with interdisciplinary teams and facility partners to drive preventive care and improved outcomes for residents and families.

Qualifications

• Current NP or PA licensure in the state of practice.
• Professional patient care experience required.
• Experience in geriatrics, family medicine, or internal medicine preferred.
• Excellent communication and ability to build trust with residents and staff across facility teams.
