Job summary

Providence Medical Group is seeking an experienced nurse practitioner or physician assistant to join a Primary Care team at Mary’s Woods, a premier senior living community in Lake Oswego, Oregon.

This opportunity offers comprehensive, relationship-centered care to an aging patient population while collaborating with a multidisciplinary team in a value-based care environment focused on improving outcomes and quality of life.

Qualifications

• Nurse Practitioner or Physician Assistant with experience in Primary Care.
• Minimum of 1 year of post-graduate Primary Care experience strongly preferred.
• Interest and comfort caring for older adults and patients with complex medical needs.
• Commitment to team-based, patient-centered care.

Responsibilities

• Combination of direct patient care and indirect patient management, including in-basket support.
• Non-paneled role focused on team-based care.
• Collaborate with Family and Internal Medicine physicians and a multidisciplinary team.
