Eventus WholeHealth in Virginia is seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to deliver comprehensive primary care in skilled nursing and senior living communities. You will conduct routine rounds and manage chronic diseases with a focus on preventive care.

You will collaborate with interdisciplinary teams and facility partners to improve outcomes and patient experiences, contributing to a supportive, patient-centered environment.

Qualifications

• Current NP or PA licensure in the state of practice.
• Experience in patient care, preferably with older adults.

Responsibilities

• Provide comprehensive primary care within skilled nursing or senior living communities.
• Perform routine rounds and proactive chronic disease management.
• Coordinate care across interdisciplinary teams and with facility partners.
• Support preventive care and improve patient outcomes and experience.
