Job summary

MDB Health Services provides medical and psychiatric services to residents in long-term care facilities across multiple states, including Tennessee. As a Primary Care Provider, you will deliver routine medical care, manage chronic conditions, and collaborate with a multidisciplinary team to support residents' health and well-being.

You will develop personalized care plans, coordinate with facility staff and families, maintain compliant medical records, and educate patients about treatment

Qualifications

• At least two years of experience is required.
• Experience providing care to geriatric populations in long-term care settings is preferred but not required.
• Excellent communication skills, especially when working with facility staff and residents' families.
• Ability to manage time effectively, including flexibility in scheduling and being responsive to patient needs.

Responsibilities

• Developing and implementing personalized treatment plans based on patient needs
• Offering medication management, including prescribing and monitoring medications
• Collaborating with a multidisciplinary team of healthcare professionals to ensure patient care
• Maintaining accurate medical documentation in line with regulatory standards
• Providing education and support to patients and their families regarding mental health conditions and treatments
