Job summary

MDB Health Services seeks a Primary Care Provider to deliver primary care to residents in long-term care facilities across Texas. You will develop personalized treatment plans, manage medications, and collaborate with a multidisciplinary team to ensure high-quality care.

This role requires strong communication with staff and residents’ families, and you will maintain medical documentation to regulatory standards while educating families on mental health options.

Qualifications

• At least two years of experience required.
• Experience with geriatric populations in long-term care preferred but not required.
• Excellent communication with facility staff and residents' families.
• Ability to manage time effectively and be responsive to patient needs.

Responsibilities

• Develop and implement personalized treatment plans based on patient needs.
• Medication management, including prescribing and monitoring medications.
• Collaborate with a multidisciplinary team to ensure patient care.
• Maintain accurate medical documentation in line with regulatory standards.
• Provide education and support to patients and families regarding mental health conditions and treatments.
