Job summary

UofL Health is seeking a dedicated APRN to provide comprehensive primary and post-acute geriatric care in long-term care and assisted living facilities. You will manage a caseload of older adults with complex multi-system conditions and support recovery through coordinated care plans.

The role requires MSN or DNP, Kentucky RN/APRN licenses, AGPCNP/AGACNP board certification (FNP considered with relevant experience), DEA and CSR, and BLS.

Qualifications

• MSN or DNP from an accredited program.
• Active, unrestricted Kentucky RN and APRN licenses.
• Board-certified AGPCNP or AGACNP; FNP may be considered with geriatric/palliative experience.
• DEA registration and CSR.
• Current BLS certification.

Responsibilities

• Conduct comprehensive geriatric evaluations and wellness exams.
• Diagnose and manage acute changes and complex chronic conditions.
• Develop and monitor post-acute rehabilitation and transitional care plans.
