UofL Health, Inc. seeks a dedicated APRN to deliver comprehensive primary and post-acute geriatric care in long-term care and assisted living facilities in Louisville, KY.

Responsibilities include managing multi-system conditions, leading post-acute care plans, and collaborating with an interdisciplinary team. Requires MSN/DNP, KY APRN license, and board certification in AGPCNP/AGACNP or FNP with geriatrics experience.

Qualifications

• MSN or DNP from an accredited program is required.
• Active Kentucky RN and APRN licenses are required.
• Board certification as AGPCNP/AGACNP; FNP considered with geriatric/palliative experience.

Responsibilities

• Perform geriatric evaluations, admission exams, and annual wellness visits.
• Diagnose and manage acute changes and complex chronic conditions.
• Develop post-acute rehab and transitional care plans.
• Prescribe medications, order imaging, and interpret labs.
• Coordinate wound care and monitor rehab milestones.
• Lead care conferences with residents, families, and representatives.
• Provide triage support to nursing staff and collaborate with the care team.
• Document EMR accurately and timely; participate in end-of-life care as needed.
