Job summary

Reliant Medical Group, part of the Optum family, seeks a physician to join our Skilled Nursing Facility team in Worcester County. This role offers a hybrid work model, combining on-site care with remote tasks when appropriate.

The ideal candidate has board certification in Internal Medicine or Family Practice, MA licensure, and geriatric experience. You will lead a multidisciplinary team focused on value-based care and patient goals, with opportunities for CME and professional development.

Qualifications

• National Board Certification in Internal Medicine or Family Practice.
• Massachusetts medical license in good standing.
• Graduate of an accredited medical school.
• Maintains BLS certification.
• Geriatrics experience or fellowship preferred.

Responsibilities

• Provide care for older adults in skilled nursing facilities.
• Lead multidisciplinary skilled rehab team including APN, nursing, therapy, social work, PT, OT, SLP, and case management.
• Monitor and manage acute and chronic conditions common in nursing homes.
• Discuss goals of care with patients and families and caregivers.
