• Experience in long-term healthcare preferred.
• Strong communication and interpersonal skills, both verbal and written.

Responsibilities

• Perform therapy treatments and functions for skilled nursing patients.
• Provide treatment plans that allow patients to reach their maximum potential or for a successful transition to the next level of care.
• Work with other therapists and nursing staff to ensure cohesive quality of care between clinical and therapy needs.
• Educate patients and caregivers to be able to succeed at home after discharge from the facility.
• Provide quality documentation as required.
