Job summary

Blue Stone Therapy seeks a Physical Therapist (PT) for Wilton Retirement Community in Wilton, IA to provide rehabilitation services to adult and geriatric residents. You will assess patients, develop plans of care, and implement treatment programs to improve gait, balance, mobility, and independence.

The role requires graduation from an accredited PT program, current licensure (or eligibility), and strong clinical reasoning, communication, and organizational skills.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license (or eligibility for licensure) in applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.
• Strong clinical reasoning, communication, and organizational skills.
• Commitment to patient-centered care and professional standards.

Responsibilities

• Perform physical therapy evaluations and develop individualized plans of care
• Provide skilled physical therapy treatment focused on functional and measurable outcomes
• Address gait training, balance, mobility, strength, range of motion, pain, and discharge planning
• Supervise Physical Therapy Assistants in accordance with licensure and regulatory standards
• Maintain appropriate patient schedules per established guidelines
• Complete timely, accurate documentation to support quality care and reimbursement requirements
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes
