Job summary

Blue Stone Therapy is seeking a Physical Therapist (PT) to provide rehabilitation services to adult and geriatric patients at St. John's Circle of Care in Springfield, MN.

The role focuses on evaluating and treating patients in skilled nursing and post-acute settings, developing care plans and implementing treatment programs to improve gait, balance, mobility, and independence. This position may serve multiple partner communities, including Divine Providence Community Home and Prairie View

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license (or eligibility) in applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.
• Commitment to patient-centered care and professional standards.

Responsibilities

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide skilled physical therapy treatment focused on functional and measurable outcomes.
• Address gait training, balance, mobility, strength, range of motion, pain, and discharge planning.
• Supervise Physical Therapy Assistants in accordance with licensure and regulatory standards.
• Maintain appropriate patient schedules per established guidelines.
• Complete timely, accurate documentation to support quality care and reimbursement requirements.
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes.
