Job summary

Westminster Communities of Bradenton is seeking a dedicated Physical Therapist to join our Health Center team. You will assess residents, plan care, provide direct treatment, and collaborate with the clinical team while contributing to program development.

The role requires an active Florida Physical Therapist license and may benefit from 1 year of long-term care experience. We offer a supportive, mission-driven environment with competitive pay and benefits.

Qualifications

• Florida PT license is required.
• One year of long-term care experience desirable; med-surg or home health helpful.

Responsibilities

• Evaluate and treat physical conditions to restore function and improve mobility; design and execute Care Plans.
• Conduct assessments and screens to determine therapy needs; provide modalities such as electrical stimulation and therapeutic massage.
• Maintain timely and accurate documentation in the systems.
• Perform Resident Home Assessments as part of the discharge process.
• Perform resident screenings during admission and quarterly/annual reviews to determine therapy needs.
