myPlace Health is seeking a Medicaid Specialist to help bring the PACE program to underserved frail seniors in Los Angeles County. You will initiate Medi-Cal applications, assist families, and ensure accurate documentation to support ongoing eligibility and Medicaid payments.

The role emphasizes serving the underserved, adhering to DHCS/CMS policies, and collaborating with the Finance and Outreach teams in a front‑line community setting.

Qualifications

• Knowledge of California Medi-Cal System and Social Security process
• 2-4 years of relevant exp
• Experience working with State/County eligibilities workers
• Experience working with Frail Elderly

Responsibilities

• Initiate and manage Medi-Cal applications for prospective participants.
• Conduct home visits to assist with Medi-Cal paperwork.
• Maintain documentation and enrollments in CRM with accuracy.
• Adhere to DHCS and CMS marketing regulations for the PACE program.
• Collaborate with Finance to prepare reports and validate Medicaid payments.
• Monitor and renew participant Medicaid eligibility.
• Provide community resources to participants and families.
• Coordinate with county/state officials to submit required docs.
• Explore AI tools to improve operations and outcomes.
• Support Enrollment and Outreach Specialist as needed.

About this Role

This is a great opportunity for an individual who is passionate about serving the underserved community in Los Angeles County. This person will have an opportunity to help bring the PACE (Program of All-Inclusive Care for the Elderly) to many underserved frail seniors in the community. This individual will help develop the process of helping frail seniors to apply for Medicaid or Medicare.

This Medicaid Specialist is a key player in keeping the underserved frail seniors continue to live at home as long as they wish. This ideal candidate brings direct experience understanding the Medicaid and Medicare new/renew process.

RESPONSIBILITIES

• Initiates, assists, and manages Medi-Cal application for prospective participants, ensuring timely and accurate completion of the process.
• Conduct home visit to complete the Medi-Cal application process. Assist participants, families/caregivers in completing all necessary paperwork needed for Medicaid re-determination or new applications.
• Maintain all documentations and record all enrollment in CRM software and ensure accuracy and compliance
• Strictly adhering to DHCS and CMS regulations and policies regarding marketing practices for the PACE Program
• Collaborate with Finance Department to prepare variety of reports for data analysis purpose. Supports the Finance department to validate Medicaid payment.
• Monitor and maintain participant’s Medicaid eligibility (recertification of Medi-Cal)
• Support the Enrollment and Outreach Specialist as needed. Provide community resources to prospective participants or families/caregivers
• Collaborate with or contact county/state government specialist to ensure the submission of required documentation or to obtain additional.
