Job summary

Mercy Health Center in Ardmore, OK seeks a Physical Therapist to develop and supervise rehab plans for adults and seniors. You will perform initial assessments, set functional goals, and coordinate care with the interdisciplinary team to deliver individualized treatment.

Qualifications

• Bachelor, Master of Science in Physical Therapy or Doctor of Physical Therapy.
• Current/Temporary Physical Therapy license in state of practice.
• BLS certification; continuing education as required.
• Prefer 6 months to 1 year rehab PT experience with adults/geriatric patients.

Responsibilities

• Perform Initial Assessment/Plan of Care per orders and policies.
• Document daily progress notes and discharge planning per policy.
• Educate patients and families on home programs and precautions.
• Collaborate with interdisciplinary team and supervise staff as needed.
• Maintain competency in CPR, licensing, and departmental requirements.
