Qualifications

• Current State Physical Therapist (PT) License
• Current CPR/BLS Certification
• Valid driver's license and reliable transportation

Responsibilities

• Evaluate residents and develop individualized care plans.
• Provide PT treatments: exercise, gait, balance, transfers, mobility.
• Reassess progress and modify plans as needed.
• Supervise PTAs per state regulations.
• Document in EMR promptly and accurately.
• Educate residents and staff on plans, exercises, fall prevention.
• Collaborate with interdisciplinary team for coordinated care.
• Maintain a productive clinical caseload with quality therapy.
