Job summary

Glengariff Healthcare Center is seeking a Physical Therapist (PT) to assess referred patients needing rehabilitation, develop and administer treatment plans, and document progress under physician direction.

The role requires graduation from an accredited PT program, valid NY license, and ability to supervise assistants. Experience in long-term or geriatric care is preferred, with strong documentation and patient-centered care skills.

Qualifications

• Graduated from an accredited Physical Therapy college or university.
• Must have a valid NY professional license and registration.
• Experience supervising Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Strong documentation writing skills.
• Experience with evaluations, progress notes, recertifications and discharge documentation.
• Collaborates with all disciplines for patient-centered care.
• Demonstrates exceptional patient service and clinical excellence.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals based on patient capacity and tolerance.
• Document patient condition and educate patients and families in physical therapy methods.
