Job summary

Glengariff Healthcare Center is seeking a Physical Therapist (PT) to assess referred patients, design and implement rehab programs, and document patient progress. The role includes supervising Physical Therapy Assistants and collaborating with the care team to deliver patient-centered outcomes.

Applicants must have graduated from an accredited physical therapy program and hold NY licensure/registration; prior long-term care or geriatric rehab experience is preferred.

Qualifications

• Licensed physical therapist with NY license/registration.
• Experience in long-term care or geriatric rehab preferred.
• Strong documentation and charting skills.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer a treatment program with goals aligned to patient capacity.
• Document patient condition and educate patients and families.
• Supervise Physical Therapy Assistants and coordinate care with other disciplines.
• Maintain confidentiality and compliance with HIPAA and residents' rights.
