Job summary

Glengariff Healthcare Center seeks a Physical Therapist (PT) to provide direct patient care for sub-acute rehabilitation and long-term care in the United States. The PT will assess patients, develop individualized treatment plans, and guide rehabilitation under physician supervision.

The role requires an accredited PT degree and an active NY license, with experience in geriatric care and strong documentation.

Qualifications

• Graduate from an accredited Physical Therapy program.
• Must hold a valid NY state Physical Therapy license.
• Oversee and supervise Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Strong documentation and medical record-keeping skills.
• Conduct evaluations, write weekly progress notes, recertifications and discharge documents.
• Collaborate with all disciplines to deliver patient-centered care.
• Demonstrate exceptional patient service and clinical excellence.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment plans with goals aligned to patient capacity under physician direction.
• Document patient condition and progress; educate patients and families in therapy methods.
• Perform evaluations, progress notes, recertifications and discharge planning.
• Maintain confidentiality and HIPAA compliance; adhere to residents' rights.
