Job summary

Reliant Rehabilitation is seeking a licensed Physical Therapist to provide therapy services in Assisted Living, Independent Living, or Memory Care settings. This PRN role offers day shifts with travel between communities.

You will screen and treat residents, develop initial and ongoing plans of care, supervise PTAs and Rehab Techs, and ensure compliant billing and documentation while delivering patient-centered care and collaborating with an interdisciplinary team.

Qualifications

• Graduate of an APTA accredited school of physical therapy.
• Ability to read, write, and speak English fluently for patient care and documentation.
• Knowledge of OBRA guidelines and health care regulations.
• Proficient in documenting patient care plans and progress notes.
• Experience with supervising PTAs and Rehab Techs preferred.

Responsibilities

• Screen residents and evaluate/treat patients referred by state-approved sources.
• Develop initial and ongoing treatment plans of care and discharge plans.
• Supervise PTAs and Rehab Techs; provide clinical supervision.
• Ensure accurate, timely billing and documentation in the medical record.
• Coordinate with interdisciplinary team to deliver patient-centered care.
• Travel between communities to provide therapy services.
