Job summary

Fusion Physical Therapy Partners in Sudbury, MA is seeking a Physical Therapist to deliver evidence-based geriatric care across assisted living facilities. You’ll evaluate mobility, design personalized plans, and guide residents toward independence while collaborating with a supportive clinical team.

You’ll work with a diverse senior population, maintain EMR documentation, and participate in ongoing professional development.

Qualifications

• Graduate of a CAPTE-accredited Physical Therapy program.
• Active state PT license in good standing (or eligibility).
• Evidence-based geriatric care focus.
• Strong communication and teamwork.
• Ability to work independently with clinical oversight.
• Commitment to continuous learning and growth.

Responsibilities

• Conduct evaluations of strength, balance, mobility, pain, and functional impairments.
• Develop and implement individualized treatment plans to improve mobility and reduce pain.
• Deliver hands-on interventions including therapeutic exercise, balance and gait training, neuromuscular re-education, and manual therapy.
• Promote resident safety and fall prevention.
• Provide care across assisted living communities with strong communication to residents and families.
• Educate residents on home exercise programs for long-term independence.
• Maintain timely documentation using EMR per state and facility standards.
• Collaborate with therapists and support staff for coordinated care.
• Participate in ongoing professional development and mentorship.
