Qualifications

• Graduate of accredited therapy program.
• Must hold current NJ Physical Therapy license.
• Minimum of 2 years of related therapy experience.

Responsibilities

• Evaluate residents and develop individualized therapy plans to support mobility and independence.
• Provide hands-on therapy to help residents reach rehab goals.
• Monitor progress and adjust treatment plans for best outcomes.
• Educate, encourage, and support residents and families.
• Collaborate with nurses, physicians, and the care team for coordinated care.
• Maintain accurate documentation per standards.
