The Legacy at Boca Raton Rehabilitation and Nursing Center in Boca Raton, FL is seeking a dedicated Physical Therapist (PT) to join our rehabilitation team. You will evaluate residents, develop individualized treatment plans, and provide high-quality therapy services to help residents reach their highest level of independence and mobility.

Qualifications

• Active Florida Physical Therapist license in good standing.
• Experience in a skilled nursing facility, rehabilitation center, or healthcare setting preferred.
• Strong clinical assessment and treatment planning skills.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide therapeutic exercises, mobility training, balance training, and pain management interventions.
• Monitor resident progress and adjust treatment plans as needed.
• Collaborate with physicians, nursing staff, and interdisciplinary team members to ensure optimal outcomes.
• Maintain accurate and timely clinical documentation per regulations and policies.
• Educate residents and families regarding therapy programs and goals.
• Ensure compliance with federal, state, and facility policies and regulations.
• Participate in care planning and discharge planning processes.
• Maintain a safe and supportive therapy environment.
