The Legacy at Boca Raton Rehabilitation and Nursing Center in Boca Raton, FL is seeking a dedicated Physical Therapist (PT) to join our rehabilitation team. The successful candidate will evaluate residents, develop individualized treatment plans, and deliver high-quality therapy to improve independence and mobility.

Qualifications

• Active Florida PT license in good standing.
• Experience in skilled nursing or rehab settings preferred.
• Strong communication and teamwork skills.

Responsabilités

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide therapeutic exercises, mobility training, balance training, and pain management interventions.
• Monitor resident progress and adjust treatment plans as needed.
• Collaborate with physicians, nursing staff, and interdisciplinary team members to ensure optimal resident outcomes.
• Maintain accurate and timely clinical documentation in accordance with regulatory and facility requirements.
• Educate residents and families regarding therapy programs and treatment goals.
• Ensure compliance with all federal, state, and facility policies and regulations.
• Participate in care planning meetings and discharge planning processes.
• Maintain a safe and supportive therapy environment.
