Job summary

Outcome HC in Ohio is seeking a licensed Physical Therapy Assistant to deliver direct PT interventions under the supervision of a licensed PT, following the physician's plan of care.

You will work with subacute clients and long-term residents at King David Campus to maximize function and quality of life, participate in an interdisciplinary team, and document daily care and training with families and caregivers.

Qualifications

• Graduate of an accredited physical therapy assistant program.
• Current license in good standing in the State of Ohio.
• Demonstrates good verbal and written communication and organization skills.
• Possess and maintain current CPR certification.

Responsibilities

• Provides direct physical therapy interventions under the supervision of the licensed physical therapist and in accordance with the physician's plan of care and as defined in the state Physical Therapy Practice Act.
• Treats subacute clients and long-term residents of King David Campus according to the plan of care to maximize function, relieve pain, ensure safe homegoing, maintain maximum performance, and improve quality of life.
• Is an integral part of the interdisciplinary team including OT, ST, nursing, aides to benefit our clients/residents.
• Completes training with families, clients, residents, caregivers
• Completes daily documentation according to facility and regulatory guidelines.
