Job summary

Fortis Care is seeking a Physiatry Advanced Practice Provider (NP/PA) to round at our Savannah partner facility, evaluating residents whose pain or dysfunction blocks therapy and returning them to the rehab team ready to work.

You’ll lead a panel in post-acute geriatric care, perform peripheral joint injections and spasticity management, coordinate with PT/OT and nursing, and serve as the go-to resource for therapy readiness, mobility, and fall prevention in a busy SNF setting.

Qualifications

• Current NP or PA license in the state of Georgia (GA).
• For NPs: certification in Internal Medicine, Geriatrics, or Family Practice. For PAs: NCCPA certification.

Responsibilities

• Evaluate residents unable to progress with focused exams to identify barriers.
• Treat with peripheral joint and soft-tissue injections, spasticity management, and targeted medications.
• Build care plans with PT/OT, nursing, and the attending team to sustain gains after discharge.
• Follow through with rechecks, documentation, and escalation as needed.
• Teach as the facility's go-to resource on therapy readiness, mobility, and fall prevention.
