Job summary

You will partner with physicians, nursing, pharmacy, case management and social work to craft patient-centered plans aligned with patient goals and values.

Qualifications
- Master's Degree in Nursing or related health care degree or equivalent experience required.
- California RN license (registered) required.
- California NP certification required.
- BLS for Healthcare Professionals required.
- DEA controlled substances registration required.
- Valid driver's license required.
Responsibilities
- Provide advanced nursing care in geriatrics and palliative care across care settings.
- Collaborate with physicians, nursing, pharmacy, case management and social work to develop patient-centered plans.
- Identify age-related vulnerabilities and support decision-making and goals of care.
- Coordinate care across hospital services and community resources.
- Educate staff and precept students; participate in program evaluation and quality improvement.
