The role emphasizes collaboration with physicians, patients and families to align on care plans and ensure smooth transitions between acute and post-acute settings.

Applicants should hold a Master’s in Nursing with CA RN and NP credentials, BLS, and DEA registration, and be prepared to participate in ongoing education and quality improvement initiatives.

Qualifications

• Master's Degree in Nursing or equivalent.
• California RN license – required.
• AHA BLS for Healthcare Professionals – required.
• California NP certificate – required.
• DEA controlled substances registration – required.

Responsabilités

• Assist inpatient physicians and healthcare team in managing patients with serious, life-limiting illness.
• Collaborate with patients, families and medical team to ensure timely plan of care and communication.
• Coordinate post-acute follow-up, consultations and referrals.
• Educate patients and families on expected inpatient course.
• Document encounters and participate in quality improvement activities.
