The role delivers primary care to frail older adults across clinic, home, assisted living and nursing facility settings, with local travel and after-hours call participation. Requires MSN or PA degree, MA licensure and prescribing authority, DEA and CPR credentials, and 1–2 years of geriatrics experience.
