Job summary

Odom Rehab in Minnesota is seeking a Licensed Physical Therapist to provide outpatient rehabilitation within assisted living settings. You will assess patients, develop and update treatment plans, and collaborate with the care team to achieve functional goals for older adults.

You will deliver manual therapy, therapeutic exercises, gait training, and patient education while maintaining compliance with state guidelines and professional standards. A valid MN PT license is required.

Qualifications

• Graduate from an accredited Physical Therapy program with a DPT degree and MN state licensure.
• At least 1 year of PT experience in a healthcare setting preferred.
• Strong knowledge of state guidelines and patient care standards.

Responsibilities

• Evaluate and treat patients per state guidelines and goals.
• Establish POC for patients and reassess regularly.
• Oversee PTA treatment based on POC.
• Deliver manual therapy, therapeutic exercises and gait training.
• Educate patients and families on condition management.
• Update POC as patient care evolves.
• Develop discharge plans and document daily treatment.
• Communicate with physicians and staff about patient status.
• Maintain licensure and participate in staff meetings.
• Track continuing education credits.
