Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, developing individualized treatment plans, and delivering interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed OT in the state where care is provided.
• Experience in skilled nursing or post-acute settings.
• Ability to work onsite with a patient-centered approach.
• Strong documentation and care coordination skills.

Responsibilities

• Evaluate residents to determine therapy needs.
• Develop and implement individualized treatment plans.
• Deliver evidence-based interventions to ADLs, mobility, cognition and safety.
• Document progress in EMR and collaborate with an interdisciplinary team.
• Educate families and caregivers on care plans and home safety.
