Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. In this onsite role, you will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Must hold active OT license in good standing.
• Experience delivering occupational therapy in skilled nursing or post-acute settings preferred.
• Ability to create individualized treatment plans and monitor progress.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.
• Document progress in EMR and communicate with the interdisciplinary team.
• Educate families and caregivers on care plans and safety.
