Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role evaluates residents, develops individualized treatment plans, and delivers evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Must hold a valid Occupational Therapy license.
• Collaborate with an interdisciplinary care team to support residents.
• Develop and implement individualized treatment plans.

Responsibilities

• Evaluate residents and determine functional levels.
• Develop individualized treatment plans and deliver evidence-based interventions.
• Address ADLs, mobility, cognition, and safety.
• Document care in the EMR system.
• Educate families and caregivers on care needs.
