Job summary
Mercer Manor Nursing and Rehabilitation in Illinois seeks a skilled Occupational Therapist to lead geriatric rehabilitation at a premier skilled nursing facility. You will evaluate residents' physical, cognitive, and psychosocial abilities and develop personalized ADL/IADL programs to restore function.
Work within an interdisciplinary team, provide occupation-based therapy, educate residents and families, track outcomes, and advocate for evidence-based care.
Qualifications
- Licensed OT with current credentials.
- Experience in geriatric rehabilitation in skilled nursing.
- Ability to design and implement ADL/IADL-focused treatment plans.
- Strong interdisciplinary collaboration with care teams.
Responsibilities
- Perform detailed evaluations of residents' physical, cognitive, and psychosocial abilities and design personalized treatment plans to restore or maximize function in ADLs and IADLs.
- Guide residents through occupation-based interventions and environmental modifications to enhance safety and independence.
- Track progress, document outcomes, educate residents, families, and staff, and collaborate with the interdisciplinary team to adjust plans for optimal independence.
