Responsibilities

• Evaluate patients and develop personalized care plans focused on functional independence and safety
• Deliver care through therapeutic activities and ADL training
• Assist patients with daily tasks such as dressing, bathing, grooming, and meal preparation
• Collaborate with interdisciplinary teams to support strong patient outcomes
• Track patient progress and adjust treatment plans as needed
