Job summary

Total Care Therapy is hiring an Occupational Therapist in Mayfield Heights, OH. You will assess residents and create individualized OT plans to improve ADLs, balance, cognition, and safety in assisted living and independent living communities.

Qualifications

• Current State OT license.
• CPR/BLS certification.
• Valid driver's license with reliable transportation.
• Experience in geriatrics, home health or assisted living preferred.

Responsibilities

• Evaluate residents and develop OT treatment plans.
• Improve ADLs, IADLs, cognition and safety.
• Assess equipment and home/environment modifications.
• Reassess progress and update plans.
• Supervise COTAs per state regulations.
• Complete EMR documentation.
• Educate residents and caregivers on safety and energy conservation.
• Collaborate with interdisciplinary team for coordinated care.
