Job summary

AltaMed is seeking an Occupational Therapist to support the Rehabilitation Manager across staffing, training, administration, and regulatory compliance for our PACE centers. The OT will cover duties remotely or in-person as needed to support assessments, care planning, and IDT meetings.

You will assist with data collection and analysis, prepare reports and presentations, and participate in CQI initiatives and interdisciplinary committees to improve participant health outcomes and optimize rehab

Qualifications

• Licensed Occupational Therapist in California.
• Minimum 1 year experience with frail elderly or disabled populations preferred.
• Bilingual abilities preferred (English/Spanish/Cantonese/Mandarin).
• Post-professional education in Physical Agent Modality preferred.

Responsibilities

• Specialized OT to support Rehabilitation Manager across staffing, training, administration, project coordination, and regulatory compliance for PACE centers.
• OT covers duties remotely or in-person as needed for assessments, care planning, and IDT/Care Coordination meetings.
• Support training such as onboarding, process implementation, and competency re-training.
• Assist with data collection, data analysis, report building, and facilitating presentations.
• Support Rehab operations with CQI initiatives and interdisciplinary committee participation.
• Collaborate with discipline managers, QA teams, and other leaders to align with goals and deliver exceptional patient care.
