Job summary

Chatham Hills Subacute Care Center in Morristown, NJ is seeking a motivated Occupational Therapist to join our rehabilitation team. You will assess referred patients, develop and implement treatment plans, and educate patients and families while coordinating with physicians.

The ideal candidate has graduated from an accredited OT program, holds a valid state license, and can supervise OT Assistants. Experience in long-term or geriatric care and strong documentation skills are preferred; timely

Qualifications

Must have graduated from an accredited OT program.
Must have a valid state license/registration for the state of practice.
The OT is responsible for supervising OT Assistants.
Experience with long‑term care and/or geriatric care is preferred.
Well-versed in documentation writing and clinical record keeping.

Responsibilities

Assess referred patients to determine rehabilitation needs and potential for therapy.
Establish and administer an individualized treatment program with goals.
Set realistic and achievable goals and document patient progress.
Educate patients and families and coordinate with physicians and care teams.
Maintain confidentiality and adhere to HIPAA and related regulations.
