Job summary

The Chatham Hills Subacute Care Center seeks an experienced Occupational Therapist to join its rehabilitation team in New Jersey. The OT will assess patients requiring rehabilitation, develop and supervise treatment plans, and document outcomes under physician direction.

Responsibilities include setting realistic goals, educating patients and families, and collaborating with a multidisciplinary team. Licensure in the state and accreditation are required, with experience in long-term or geriatric

Qualifications

Must have graduated from an accredited Occupational Therapist program.
Must hold a valid license/registration for the state.
Oversee and supervise Occupational Therapy Assistants.
Experience in long-term/geriatric care is preferred.
Strong documentation writing skills.
Experience with evaluations, weekly progress notes, re-certification, and discharge documentation.
Collaborates with all disciplines to provide patient-centered care.
Excellent patient service skills and clinical excellence.
Punctual, professional, and adherent to schedules.
Perform duties assigned by the Director of Rehabilitation.
Maintain confidentiality and HIPAA compliance.

Responsibilities

Assess referred patients to determine rehab needs and capabilities.
Develop and administer individualized treatment plans with goals.
Document patient condition, progress, and outcomes; educate patients and families.
