Job summary

Chatham Hills Subacute Care Center in Hillside, NJ is seeking an Occupational Therapist to join our rehabilitation team. You will assess referred patients, develop and implement individualized treatment programs, and document outcomes under physician direction.

The role requires licensure, and experience with long-term or geriatric care is preferred. You will collaborate with an interdisciplinary team to deliver patient-centered care and ensure timely progress notes and discharge planning.

Qualifications

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Evaluations, weekly progress notes, re-certification and discharge documentations.

Responsibilities

• Assess referred patients and develop rehabilitation treatment plans.
• Establish and administer treatment goals aligned with patient capacity.
• Educate patients and families in therapy methods and document progress.
