Job summary

Yukio Okutsu State Veterans Home in East Hawaìi is seeking a dedicated Occupational Therapist to join our interdisciplinary team. You will provide evidence-based rehabilitation to residents, developing individualized plans to promote independence and safety in daily activities.

Responsibilities include comprehensive evaluations, ADL/IADL interventions, adaptive equipment recommendations, and thorough documentation.

Qualifications

Graduate of an accredited Occupational Therapy program.
Current Hawaii OT license in good standing.
NBCOT Certification as an Occupational Therapist Registered (OTR).
Current Basic Life Support (BLS) certification.
0–4 years of clinical OT experience; geriatric rehab preferred.
Knowledge of Medicare guidelines, EMR usage, and strong communication and teamwork.

Responsibilities

Perform comprehensive occupational therapy evaluations and develop individualized treatment plans.
Deliver evidence-based interventions to improve ADLs, IADLs, upper extremity function, cognition, coordination, and overall functional independence.
Recommend adaptive equipment, assistive devices, and durable medical equipment; educate residents, families, and caregivers.
Maintain accurate, timely documentation and may supervise COTAs; adhere to regulatory standards.
Collaborate with the interdisciplinary care team on care and discharge planning; participate in quality outcomes.
Promote resident safety by infection prevention and continuing professional development.
