Job summary

Yukio Okutsu State Veterans Home in East Hawaiʻi is seeking a Certified Occupational Therapy Assistant (COTA) to deliver compassionate, evidence‑based OT services under supervision. The COTA will help residents maximize independence in daily activities, collaborate with a multidisciplinary team, and develop clinical expertise in a skilled nursing setting.

The ideal candidate has an OTA associate degree, Hawaii license, NBCOT certification, and BLS.

Qualifications

Associate degree from an accredited COTA program.
Current Hawaii Occupational Therapy Assistant license in good standing.
NBCOT certification as a COTA in good standing.
BLS certification.
0–4 years of clinical COTA experience; SNF/geriatric rehab preferred.
Knowledge of Medicare guidelines, EMR, and strong communication and teamwork.

Responsibilities

Implement OT treatment plans to improve ADLs, IADLs, upper extremity function, cognition, and independence.
Monitor progress and assist with evaluations, reassessments, discharge planning, and equipment recommendations.
Educate residents and families on treatment plans, activities, and progress.
Complete accurate clinical documentation meeting CMS and facility standards.
Collaborate with the interdisciplinary team to support safe discharge and positive rehab outcomes.
Promote safety through infection prevention and regulatory compliance.
