Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role evaluates residents, develops individualized treatment plans, and delivers evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed OT in good standing (state licensure).
• Experience in geriatrics or SNF preferred.
• Ability to work onsite and collaborate with team.

Responsibilities

• Evaluate residents and develop treatment plans.
• Deliver interventions to improve ADLs, mobility, cognition, safety.
• Document in EMR and coordinate with families and caregivers.
