Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. In this onsite role, you will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Must hold an OT license in good standing.
• Experience with seniors in skilled nursing or post-acute settings preferred.
• Onsite role with EMR documentation and family education.

Responsibilities

• Evaluate residents and assess functional abilities.
• Develop individualized treatment plans and goals.
• Deliver evidence-based interventions to improve ADLs, mobility, cognition and safety.
• Document in EMR and communicate findings.
• Collaborate with an interdisciplinary team.
• Educate families and caregivers.
