Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, developing individualized treatment plans, and delivering evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed OT with current licensure in the state
• Experience in skilled nursing or post-acute settings preferred
• Ability to develop individualized treatment plans and goals

Responsibilities

• Evaluate residents in SNF/post-acute facilities.
• Develop and implement individualized treatment plans.
• Deliver evidence-based interventions to improve ADLs, mobility, cognition and safety.
• Document assessments and progress in EMR.
• Collaborate with an interdisciplinary team and educate families and caregivers.
